Overview

Chart Squad is a PCH Partner that streamlines the collection of medical records by leveraging patient access requests. This removes the burden from Case Managers and Paralegals of manually chasing providers for records.

How it works

  • Patient authorization: Uses HIPAA-compliant patient right-of-access to request records directly from providers.
  • Automated follow‑ups: Systemized reminders and escalation to reduce delays.
  • Delivery: Records are delivered back to PCH and associated with the correct case.

Using Chart Squad from the Dashboard

  1. Identify cases needing records (e.g., treatment complete, billing required, demand prep).
  2. Initiate a records request via the partner action (if enabled for your role).
  3. Provide necessary details (provider, date range, record types).
  4. Monitor status; records will attach to the case when received.

Tips

  • Be specific: Include precise date ranges and document types to avoid rework.
  • Check status: Use the case’s records section to track incoming documents.
  • Coordinate: Pair with Billing/Medical Records teams for downstream tasks.

Troubleshooting

  • No response from provider? Ensure contact info is correct and that patient authorization is on file; escalation steps may be required.
  • Records attached to wrong case? Move or re-index via the records management workflow and note the correction in the case timeline.
